Physical Activity Readiness Questionnaire

For most people physical activity should not pose any problem or hazard. PAR-Q is designed to identify the small number of adults for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them.


The questionnaire is suitable for those aged between 15 and 69. If you are over 69 years of age, and you are not used to being very active, please check with your doctor. Common sense is your best guide in answering these questions.


Read the questions carefully and answer each one honestly.


Has a doctor ever told you that you have high/low blood pressure?

Have you ever been diagnosed with a heart condition or told that you should only ever do physical activity recommended by a doctor?


Do you have diabetes or any other metabolic condition?


Has your doctor ever said you have a bone or joint problem which is or could be aggravated by exercise?


Have you ever felt pain in your chest during physical activity?


Are you taking any medication?


Have you ever suffered from unusual shortness of breath at rest or with mild exertion?


Do you have any physical disabilities of any kind?


Are you or do you think you might be pregnant?


Have you been in hospital in the last 2 years?


Have you had any surgery or major illness in the last 6 months?


Are you undergoing treatment or having any regular checks made for anything at your doctors, a hospital, or a clinic?


Do you ever feel faint, have spells of dizziness, or have lost balance or consciousness?


Do you know of any other reason why you should not participate in an exercise class or training programme?

If you answered yes, to one or more questions:


If you have not recently done so, consult with your doctor by telephone or in person before increasing your physical activity. Tell your doctor what questions you answered ‘yes’ to on PAR-Q or present your PAR-Q copy.


After medical evaluation, seek advice from your doctor as to your suitability for:


· Participation in a high impact and high intensity aerobic and exercise class

· Unrestricted physical activity starting off easily and progressing gradually

· Restricted or supervised activity to meet your specific needs, at least on an initial basis


If you answered no to all questions:


If you answered PAR-Q accurately, you have reasonable assurance of your present suitability to increase your physical activity level, to complete a graduated exercise programme and to participate in high impact and high intensity anaerobic and aerobic exercise.


Assumption of Risk


I hereby state that I have read, understood and answered honestly the questions above. I also state that I wish to participate in activities that may include aerobic exercise, weightlifting exercise, and stretching with and without additional equipment (e.g. weights, steps, hurdles).

I state that I can identify exercises which may cause me pain and will not engage in these. I realise that my participation in these activities involves the risk of injury. If the answer to any of the above questions’ changes, I will consult my doctor, and inform the coach as necessary.

I have read, understood and completed this questionnaire answering to the best of my knowledge.
